Child Care Registration Form

Child care services will be provided at APA's 1998 annual meeting in Toronto from Sunday, May 31, to Thursday, June 4, for children ages 6 months through 12 years. Sign up in advance to assure a space for your child/children. The preregistration deadline is May 1.

Please list each child's name and age below. Indicate the reservations you would like to make for each child within each time frame. Children must be registered for a minimum of three consecutive hours per child.
